The need to repeatedly solve the same problem usually indicates the real root cause was not addressed or implementation of the initial solution was not well defined and/or executed.
Solution Path
The solution path for this issue can vary depending on the problem’s complexity and the definition of the business process associated with the problem.
The basic process associated with the problem must be defined. Once the process is understood, the EOS IDS tool can be used to work toward a solution.
A more structured root cause analysis might be required for complex problems. In this case the evaluation would include reviewing the process performance standards, inputs, resources, procedures, training/knowledge needs and required outputs.
Corrective actions can be undertaken once the root cause is determined. These actions must include documentation, training, monitoring, and clear accountabilities to ensure the problem stays resolved.
